• Vital Statistics Check List: 
  Use this check list to mail in your request for a copy of your original 
  birth certificate, marriage license or a family member's death certificate.

1. A letter requesting the original document (birth certificate, marriage 
   license, ect.) from and signed, by you personally.
2. The letter must contain; your full name as printed on
   your birth certificate, the names of both your parents, and your contact 
   information.
3. A xerox copy of an ID card of the
   state where you currently reside 
4. A xerox copy of your Passport
5. A xerox copy of your Soc. Security Card (Optional)
6. Lastly include a money order of $7.00 addressed to the American Samoa
   Government, Vital Statistics Office
